About the role

You'll serve as the Primary Care Team RN within a virtual primary care team. As we expand our Oscar Primary Care offering to develop interdisciplinary care teams for a virtual patient centered medical home, the RN will provide and coordinate patient care.

You'll collaborate with your Care Teams' medical providers, operations, product teams, and concierge teams to support patient care virtually.

You will report to the Medical Director, Virtual Primary Care.

This is a remote / work-from-home role. You must reside in one of the following states: Arizona, California, Colorado, Florida, Georgia, Nevada, New York, Ohio, Pennsylvania, Texas, or Virginia. Note, this list of states is subject to change. #LI-Remote

The base pay for this role is: $72,000- $94,500 per year. You are also eligible for employee benefits and annual performance bonuses.

Responsibilities

Conduct ad hoc and scheduled telephonic, video or asynchronous messaging visits with empaneled patients and document encounters in order to:
- Triage and assess new and established patients' needs
- Follow up on patient's Care Plan adherence, and work with the patient's primary care provider to make Care Plan changes as necessary
- Follow and support the management of patients' chronic conditions per direction of MD or APP or under standardized protocols/procedures
  - May include diabetes, hypertension, depression management, among others
- Conduct post acute or post visit symptom checks via telephone or video
- Provide education, counseling, and support healthcare goal setting
- Check in with patients on regular basis according to their acuity; higher acuity members receive more regular check ins (e.g. those who are more medically complex, have multiple admissions, lack of social support, low health literacy, several barriers to SDoH)
- Care coordination for patients with medical or behavioral health needs
Triage patient symptoms and requests and serve as a clinical escalation point for other care team members (Medical Assistants, etc)
High level oversight of patients transitions of care:
- Perform facility call while patient is inpatient to review transition to home plan
- Coordinate Post-ER and hospital discharge appointments and specialist visits as indicated
- Conduct Medication Reconciliation post discharge
- Have consistent communication and relationships with Hospital Case Managers in Care Team's region
- Coordinate with other care team members to ensure completion of outstanding tasks post discharge (e.g. follow up appointments, ensuring ordering of DME or additional tests)
- Reassess ongoing care needs 30 days post discharge
Manage pre-visit offerings as appropriate by patient type and/or disease status
- Collect vital signs, medical history and screening surveys prior to provider visits
Support development of expanded nursing services by providing feedback on:
- Triage protocols
- Software development to support nursing care
- Expanded nursing services within the scope of practice
Adherence to all member privacy and confidentiality standards of practice (i.e. HIPAA)

Qualifications

Active, unrestricted multistate RN license.
3+ years of outpatient / ambulatory nursing experience
Working knowledge of Milliman Care Guidelines

Bonus Points

1+ years experience in basic behavioral health interventions used in primary care
1+ years experience in virtual based practice
Spanish language fluency

Pay Transparency:

Final offer amounts, within the base pay set forth above, are determined by factors including your relevant skills, education, and experience.
