This is an OPS (Other Personal Services) positions and as such no paid time off is offered. Health benefits will be offered for full-time employment. OPS employees are eligible to participate in a deferred compensation plan.

Direct Public Health Nursing Care:
Serves as Subject Matter Expert on community committees including but not limited to Child Abuse Death Review, Fetal and Infant Mortality Review, Domestic Violence Fatality Review and Healthy Start

Conducts outreach, presentations and attends community events.

Provides consultation to the Healthy Start Risk Screening Office

Responsible for Florida Healthy Babies Workplan and other workplans as appropriate.

Provides public health advanced level assessment of health status and health care needs of individuals, families and/or groups of individuals (particularly those with complex and multiple problems and communicable disease) in a variety of settings, including clinics, schools and other public/community facilities. Provides in-depth health teaching, guidance, counseling and advisement that is directed toward the promotion and maintenance of optimal health of individuals, families and groups of individuals. Formulate and initiate comprehensive care management for individuals and/or families served which also includes follow-up.

Participates in conference calls and trainings and submits monthly reports.

Maintains records and reports, documenting service and care provided as required. Participate in educational, professional and community activities that relate to professional growth and aid in the improvement in the delivery of client care.

Responsible for coding Employee Activity Record (EARS). Reviews/responds to emails daily and complete on time all administrative requirements such as EAR’s and timesheets.

Protect the security of medical records/HER to ensure that confidentiality is maintained.

Attend all mandatory meetings and trainings.

Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special need: or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.

Performs other related duties.

MINIMUM QUALIFICATIONS:
Maternal Child Health experience.
Licensure Registered Professional Nurse in accordance with Chapter 464, Florida Statutes

Preferred Qualifications:
Conducts outreach, presentations and attends community events.
