Receives referrals from physicians and facilities to initiate the intake process.
Runs verification and eligibility of insurance and uploads to patient’s chart.
Adds new patients into the EMR and enters all information required to create an episode and track patient information.
Coordinates with the Scheduling Coordinator to ensure all disciplines are staffed and assigned to each chart in the EMR.
Receives medical documentation to scan into the system, following the company’s scanning policies.
Obtains initial and on-going authorization for service requests from non-Medicare pay sources. Ensures authorization for services are in place for the ordered treatment periods.
Coordinates with the Clinical and Marketing teams to ensure required documentation is obtained from physicians.
Attends regular interdisciplinary team meetings for purposes of identifying new admissions and recertifications.
Reviews the monthly eligibility report and ensures that all insurance corrections are made to patient demographic information.

At least 1 year of home health intake experience (preferred)
Excellent working knowledge of primary home health diagnoses, related interventions and care protocols.
Demonstrates working knowledge of Medicare and non-Medicare reimbursement requirements including eligibility for service, managed care and Veterans Administration pre-authorization processes, and Medicare managed requirements.
Ability to quickly learn new software applications.
Computer skills for Google Docs, Microsoft Office, electronic medical records and accessing G-mail.
Basic medical terminology.
Responsibilities
Manages and oversees patient referrals.
Process Initial Home Health Order to ensure patient receives care within 48 hours.
Coordinates and oversees department functions, duties, and activities for Intake of new referrals.
Acts as a liaison between LORIAN and referral sources to ensure smooth processing of referrals
Ensures that all relevant patient information is obtained in an accurate and timely manner, and is entered into HomeCare HomeBase system, in order to meet MEDICARE requirements or other insurances, when patients are referred to LORIAN by referral sources.
Regularly collaborate with the Sales and Clinical management team in reviewing new referrals and determining the appropriateness of any given referral for the agency.
Provides all relevant patient information (i.e. patient personal demographic, history and physical, current medications, physician’s orders, Face to Face (F2F), surgical reports, etc.) obtained upon referral to the Medical Records Department for scanning into patient charts.
Identify, assign and schedule the required Start of Care clinical staff to each patient admitted by LORIAN.
Assists in patient concerns and immediate needs of the patient and delegates to clinical staff as necessary.
May perform other duties as assigned by management staff.

Required Education and Experience:
High School education
One (1) to Two (2) years of experience in health care, home health preferred.
Must have professional and customer-service-driven phone skills
Communicates effectively with all customers internal and external in a constructive, goal oriented, and professional manner.
Ability to be flexible, to follow verbal and written instructions, and to work in a team environment.
Strong knowledge of Microsoft Office Systems (i.e. Word, Excel, PowerPoint, etc.) and computerized systems in general required.
Proficient in completing routine paperwork, multi-tasking, and providing appropriate follow-up as needed.
